Registration Form for the Senior pennant 2012 Summer

Individual or Team applications MUST be received by Monday, 30 January 2012.

The DVTTA entry for the season is $30.00 per player (or $90.00 per Team), this amount MUST be paid on the FIRST night of competition.

NOTE: Only financial players will be permitted to play. Nightly table hire is $32.00 (payable at the Centre's Reception desk), which is $5.20 for each player.

The competition is expected to start the week commencing Monday, 6 February 2012.

The Committee reserves the right to reject entries. No player can be guaranteed a game if this Registration Form is received LATE or a player fails to submit the form below.
